Health Benefits
SBL offers a comprehensive health & prescription plan, administered by Aetna. Full-time and part-time employees are eligible to enroll in individual, employee & spouse, employee & children or family coverage. The plan offers two networks / tiers of coverage. The SBL Network includes all SBL facilities, employed providers and many other staff members. The Aetna Network is a more comprehensive group of providers and facilities contracted through Aetna.
Dental
Benefits:
Maintaining
healthy teeth and gums and seeking professional treatment when dental
problems arise is important to your overall health. The Sarah Bush
Lincoln Health System Dental Plan offers coverage to aid in your
dental health.
Vision
Benefits:
As a
participant, you and your dependent may receive the following vision
benefits:
Examination - Every 12 months - $40 paid
Glasses or
Contact Lenses - every 24 months - $150 paid
Health Care Reimbursement Account:
The flexible benefit plan offers you a choice to participate in comprehensive health, dental and vision plans. However, as with any plan, there are some items that are not covered or are only partially covered. The HCRA is designed to let you take advantage of current tax laws and pay for these out-of-pocket expenses with pre-tax dollars. Any money left in your account, by law, must be forfeited.
Dependent Care Reimbursement Account:
The flexible benefit plan also offers you the opportunity to pay for dependent care expenses on a pre-tax basis.
Long-term
Disability:
Sarah
Bush Lincoln Health System will provide full-time employees with
basic Long Term Disability coverage at no cost. If you become totally
disabled, the basic LTD Plan will pay 60 percent of your base
earnings up to a maximum benefit of $6,000 per month. Payments will
begin after you have been disabled for 90 days.
Short-term
Disability:
As a
full-time employee, you have the opportunity to purchase Short Term
Disability coverage to protect your income from the end of your
waiting period to the 90th day when Long Term Disability payments
begin. You can choose from two options:
Coverage beginning
after 14 days of disability
Coverage beginning
after 30 days of disability
Employee
Life Insurance:
Full-time
employees are provided life insurance coverage. You will receive a
standard life insurance benefit of two times your annual salary. The
benefit is provided at no cost to you and will be paid to your
designated beneficiary in the event of your death.
Voluntary
Term Life Insurance:
You
may choose up to $250,000 of Voluntary Term Life Insurance. The additional life insurance you purchase
through the Plan is portable which means that even after your
employment with SBLHS, you may continue participation by paying your
premiums directly to the insurance company.
