Connecting to your health information is as easy as 1, 2, 3!
1. Complete
registration online or at the clinic to become a Patient Portal user.
2. Present your
photo ID at your clinic or in Medical Record Management at the Health Center.
3. Sign the
Terms of Agreement
The security of your health information is important to us. To ensure confidentiality, we request the Patient Portal user present a photo ID and sign the Terms of Agreement.
You will receive your user ID and password through your personal email account.
If you would like to manage your child’s account, or that of another person, you can do so by completing a proxy authorization. If you have questions or need assistance, please ask your clinic staff. They are happy to help you. You may also call Medical Record Management at 217 258-2536.
