Clinical Significance
Fentanyl may be used as an adjunct to surgical anesthesia (intravenously or intramuscularly), in the management of chronic pain (transdermal patches) or for pain associated with cancer (oral, transmucosal). High analgesic potency and the ability to induce euphoria have lead to experimentation and abuse of fentanyl. Fentanyl (Sublimaze®, Duragesic®,Actiq®) is a synthetic narcotic analgesic capable of depressing the central nervous system and producing sedation, muscle relaxation, drowsiness, respiratory depression, hypotension and coma. Reports of fentanyl misuse have been associated with individuals that have a history of opiate/opioid abuse and with individuals that work in the healthcare profession. The euphoric effects of fentanyl are similar to those of other opiate/opioid such as codeine, heroin, hydrocodone, morphine, and oxycodone. Fentanyl has high potential for abuse and must be monitored closely to prevent diversion into non-medicinal uses.
