This is a custom designed exercise and lifestyle program to help individuals achieve their goals and manage chronic illness. The program features a free information session, health-related fitness testing, and an individualized and supervised exercise program. The overreaching goal is to instill lifelong healthy behaviors that foster independence and encourage participants to transition to a long-term community based program. If a decision to join the program is made, an extensive health history will be completed and additional appointments will be made including the initial physical fitness assessment. Payment will be due at the initial physical fitness assessment appointment.
