Patient Rights and Responsibilities
As a patient, you have rights. Please take a moment to read the following information.
Your rights are important to us
There are laws in place that are designed to protect you as a patient. These patient rights include — but are not limited to — privacy practices and informed consent. If you need medical care, your health care provider must provide you with the information you need to make an informed decision. Also, your privacy must be secured, which includes medical records as well as personal privacy.
As a patient under our care, Effingham Ambulatory Surgery Center makes every effort to ensure your rights are respected, honored, and protected. The attached links are to documents that —
- provide you with your rights and responsibilities relating to your surgery.
- describe how to file a grievance, if desired.
- provide information concerning physician ownership of our center.
- set forth our center’s policy with respect to advance directives.
Nondiscrimination Notice
We invite you to learn more about your rights and responsibilities as a patient and take charge of your healthcare experience.
